The service

Her Form is operated by PT FORM LIFE BALI and provides at-home blood collection, laboratory analysis by accredited partner labs, and specialist review of hormone biomarkers.

Not a substitute for emergency care

Her Form is preventive and functional testing. It is not an emergency service. If you are experiencing acute symptoms, contact local emergency services or your doctor.

Booking & cancellation

You can reschedule a home visit up to 24 hours before the appointment at no cost. Cancellations within 24 hours may incur a nurse call-out fee.

Payment

Payment is taken at checkout through Shopify in IDR, AUD or NZD depending on your selected market. A 2% service fee covers card processing and home-visit logistics.

Liability

Our liability is limited to the value of the package purchased. We do not accept liability for decisions you make about your health outside of our health specialists's written protocol.
